Job Description
Colonial Behavioral Health provides opportunities for recovery to individuals and their family members in the areas of mental illness, substance use disorder, and developmental disabilities. Colonial Behavioral Health serves the citizens of James City County, City of Poquoson, City of Williamsburg, and York County.
We are currently recruiting for a Case Manager II position within our Development Disabilities program. This position will provide comprehensive case management services to individuals with Developmental Disabilities.
Job Duties
Job Responsibilities
- Develops an Individualized Service Plan (ISP) based on assessment information in collaboration with the individual.
- Monitors progress, service delivery, well being, and satisfaction by providing service to all individuals on assigned caseload.
- Assists the individual directly with obtaining needed resources.
- Other duties as assigned
Work Schedule
Monday through Friday work schedule.
(This position may require flexing of work schedule to accommodate consumer need)

Skills and Qualifications
- Knowledge of case management duties and responsibilities.
- Must be able to maintain a high level of confidentiality.
- Possess the ability to interact with clients, coworkers, and the public in a pleasant, tactful and courteous manner.
- Effective communication skills.
- Bachelor’s degree in the Behavioral Sciences or related field required.
- Qualified Developmental Disability Professional (QDDP) required.
- Valid Driver’s license and acceptable driving required.
- Experience in case management in a community setting and experience working with the dual diagnosed (DD/MH/SA) is preferred.
