Job Description:

Park Forest Care Center, Inc. is a long-term care facility specializing in behavioral health residents. As the Social Services Director, you will play a pivotal role in managing resident care and ensuring their overall well-being within our facility. Additionally, you will oversee Deer Oaks Behavioral Health Services for the residents in the community.

Responsibilities:

1. Resident Care Management:

Oversee the assessment and evaluation process for all residents, including PASRR (Preadmission Screening and Resident Review).

Maintain accurate and up-to-date documentation of reportable occurrences, ensuring compliance with relevant regulations and guidelines.

Coordinate and participate in care conferences to develop and update individualized care plans for residents.

Manage the Utilization Limitation Tracking System (ULTC) to ensure appropriate utilization of services.

2. Deer Oaks Behavioral Health Services:

Coordinate and manage Deer Oaks Behavioral Health Services for the residents in the community.

Collaborate with Deer Oaks providers to ensure residents receive appropriate behavioral health support and treatment.

Monitor the delivery and effectiveness of behavioral health services, making necessary adjustments as needed.

Maintain open lines of communication with Deer Oaks providers to address any concerns or issues promptly.

3. Ancillary Services and Admission/Discharge Management:

Collaborate with ancillary service providers (e.g., therapy services, hospice care) to ensure residents receive necessary support and treatment.

Facilitate the admission and discharge processes, including coordinating with other departments and external agencies as needed.

Maintain comprehensive records of admission and discharge information for accurate reporting and tracking.

4. Regulatory Compliance and Auditing:

Conduct regular audits of the MOST (Medical Orders for Scope of Treatment) forms to ensure compliance with resident preferences and advance care directives.

Monitor and manage psychotropic medication administration, ensuring adherence to regulations and guidelines.

Coordinate and oversee the management of assisted devices and provide recommendations for PASRR (Preadmission Screening and Resident Review).

5. Resident and Family Support:

Address and resolve resident grievances and complaints promptly and effectively.

Facilitate and participate in the Family Council, fostering open communication and collaboration between families and the facility.

Assist in managing residents' challenging behaviors by implementing appropriate interventions and coordinating with the interdisciplinary team.Quarterly Care Conferences required. Serving as a residents advocate to resolve any and all grievances.

6. Social Services and Administrative Tasks:

Respond to social services requests from residents, their families, and other staff members in a timely and compassionate manner.

Manage and process PETIs (Patient Eligibility Tracking Inquiry) in accordance with applicable regulations and guidelines.

Follow up on appointments, ensuring residents receive necessary medical, psychiatric, or therapeutic care.

7. Psychotropic Medication Management: Collaborate with the healthcare team to monitor and manage psychotropic medications for residents, ensuring appropriate usage and adherence to regulations.

8. Behavioral Education for Employees: Oversee and coordinate education on behavioral services for residents and employees, including the development and implementation of individualized behavior management plans. Collaborate with the interdisciplinary team to monitor and assess residents' behavioral needs and progress, and make appropriate adjustments to their care plans.

Requirements:

Bachelor's degree in Social Work or a related field (Master's degree preferred), or equivalent experience in a relevant role.

Valid licensure or certification as required by the state.

Proven experience in a similar role within a long-term care or behavioral health setting.

Experience managing Deer Oaks Behavioral Health Services or similar behavioral health services in the community.

Strong knowledge of relevant regulations, guidelines, and best practices.

Excellent communication and interpersonal skills to interact with residents, families, staff, and external stakeholders.

Ability to multitask, prioritize responsibilities, and work effectively in a fast-paced environment.

Proficiency in maintaining accurate and detailed documentation.

Compassion, empathy, and a genuine commitment to providing high-quality care to behavioral health residents.

Job Type: Full-time

Pay: $28.00 - $38.00 per hour

Expected hours: 40 per week
