RN Care Manager to plan and prioritize care for individuals across the life span, focusing on strategies that will promote optimal health. Working as an integral part of the primary care team for a physician owned, fast paced internal medicine and pediatric practice. Demonstrates expertise, current knowledge in nursing care and management of a caseload of patients of varying complexity and seeks to improve patient, family, and health systems / community outcomes through the application of educational concepts/skills and preventive care.

Essential Functions

• Develop plans of care and makes recommendations to PCPs, specialists and other members of the health care team regarding care management strategies, identifying strategies to maximize continuity of care across the continuum
• Communicates and collaborates with patient/family/caregivers/providers/payers to coordinate services that improve access to appropriate services across the continuum of care, and which promotes optimal health in a cost-effective manner
• Assesses the educational needs of patients, families and members of the health care team and develops and implements appropriate teaching strategies or makes appropriate referrals
• Assesses internal and external referrals to identify patient/family needs, level of intensity, insurance benefits and other patient resources.
• Measures patient care outcomes, interprets reports and analyzes data trends for groups of patients
• Documents patient data, plan, interventions, and outcomes according to practice guidelines

Requirements: Graduate of accredited Registered Nursing program required,

• Minimum 3 years of clinical practice experience required
• Experience in acute care or ambulatory care settings preferred
• Licensed RN in the State of Michigan required
• Current BLS for the Healthcare Provider by 90 days after hire required
• Employer sponsored training in Care Management by 90 days after hire required

Job Type: Full-time

Pay: $29.00 - $38.00 per hour

Expected hours: 20 – 36 per week
