Position Overview:
The primary purpose of the Resident Care Coordinator (RCC) is to utilize expertise in Geriatric Care while working under the supervision of the Resident Care Team Manager, to operate proactively by sustaining a positive, well-organized community for the residents.

This position provides support to Caregiving staff and may be assigned to Caregiver shifts when needed.

Key Responsibilities:

  • Implement ADL’s/Temporary Service Plans with changes to plan of care
  • Audit dashboard/medication pass/alert charting/ADL’s
  • Assist with updated care plans as needed/annually and ensure changes are communicated with care staff and family while ensuring care staff sign to acknowledge changes
  • Train care staff on changes to systems including expectations, implementation, and efficiency
  • Assist with communication with physician and pharmacy for implementing new orders and discontinued orders
  • Assists with training medication technician on reviewing medication changes.
  • Assist with resident care and medication pass as needed
  • Train staff on alert charting/observations and follow up with proper training as needed
  • Assist with care conferences with residents and family members as needed and annually
  • Assist with auditing and thinning charts for needs and archiving
  • Pass medications often to observe needed changes for efficiency
  • Assists with programing medication pass times to create a routine and follow-up for needed with corrections and changes
  • Assist with following up and completing incident reports
  • Work with outside agencies for coordination of care with each resident as needed
  • Assist with conducting interviews for caregiver and med tech positions as directed
  • Provides day-to-day oversight of care team, reporting all significant issues to DNS for direction
  • Specific RCC’s have been designated as the trainer for HCA-in-Training program and Dementia/Mental Health programs as directed by Executive Director.

Qualifications:

  • Education and/or Experience: High school diploma or general education degree (G.E.D) preferred; Minimum of 18 years of age
  • Credentials as Required by DSHS:
    • Washington State License Requirements: Current Certified Nursing Assistant license OR Current Nurse Assistant Registered license with proof of work prior to 2012 OR Current Home Care Aide license
    • Dementia and Mental Health specialty training within 90 days of hire
    • Current CPR and First Aid certification or ability to obtain within 30 days of hire
    • Washington State Food Worker Card or the ability to obtain within 14 days of hire
    • 12 Hours of Continuing Education Units each year by birthday
    • Nurse Delegation and Insulin Delegation required

  • Abilities:
    • Proficient at reading, writing, and communicating in English
    • Ability to occasionally lift, pull, or push up to 50 lbs.
    • Able to walk and/or stand up to 80% of the work day
