Mount Sinai Rehabilitation Hospital a leader in Behavioral Health Care has an amazing opportunity for a Primary Therapist / Social Worker who has a desire to provide a comprehensive and compassionate care to our patients.
The Primary Therapist serves as the driver of a collaborative, interdisciplinary process which coordinates, monitors, and plans behavioral health care services to meet the individual needs of our patients. The Primary Therapist incorporates case-management functions into the daily activities by acting as a resource for the entire care delivery team, patients, and their families, by monitoring, evaluating, and integrating direct clinical and supportive services.

The Primary Therapist incorporates case-management functions into the daily activities by acting as a resource for the entire care delivery team, patients, and their families, by monitoring, evaluating, and integrating direct clinical and supportive services.
What Will You Do
- Completes appropriate documentation for intake, assessment, treatment planning and discharge.
- Collaborates with clinical program staff to ensure appropriate treatment options are explored.
- Establishes relationships with patients and families to set reasonable expectations for their care.
- Completes all aftercare referrals and collaborates with outside agencies as needed.
- Demonstrates problem solving, conflict resolution, and positive communication through teamwork.
- Education: master’s degree in behavioral health field from an accredited college or university required
- CT licensure in appropriate professional designation preferred, i.e. Licensed Master Social worker, Clinical Social Worker, Psychologist, Marriage and Family Therapist, or Professional Counselor.
- Previous behavioral health experience or internship work preferred
- American Heart Association – Healthcare Provider CPR or American Red Cross CPR, Professional Rescuer required / provided upon hire
- Full Time, 40 hours
