Overview

The role of the Quality of Life (QOL) - Social Work is to create an environment in the home that provides opportunity for the community members to enjoy the highest possible quality of life and provides psychosocial support to elders, families, and partners, which will enhance the quality of life for elders and will promote the health of the intentional community of home.

Pay Range: $18.00 to $22.00 per hour based on experience

Responsibilities

  • Provides ongoing mentoring of the elder assistants in diversion and recreational enrichment.
  • Know each elder assistant in every house including their LTC experience and have a familiarity with his or her work style and personality.
  • Know each elder and their family including a psychosocial profile of the elder and an understanding of the family members and dynamics.
  • Completes an in-depth assessment of each elder and provides for elder specific materials for activities identified by the elders.
  • Participates in care planning for elder needs.
  • Assures that out of home activities are available to elders.
  • Includes families in the planning process.
  • Evaluates the effectiveness of the programs and activities.
  • Weekly visits to each home to evaluate individual levels of engagement.
  • Create individual opportunities for all newly admitted elders as per home activities Policy.
  • Documents in accordance with the organization’s policies.
  • Complete the mini mental upon admission.
  • Initiates upon admission inventory of elder’s belongings.
  • Performs any other duties as assigned.

The following are skills needed to perform, with or without reasonable accommodation, the essential functions of providing care to elders. The ability to:

  • Recognize the autonomy and dignity of all elders.
  • Communicate a sense of caring, concern, and dignity for elders.
  • Understand how to place decision-making in the hands of the elders whenever possible and appropriate.
  • Make prompt and accurate judgments with regard to elder care and emergencies.
  • Work and communicate effectively as part of the self-directed work team.

Qualifications

  • Education: Associate degree, Social Services preferred. Post-secondary education preferred

  • Licensure: OH Licensed Social Worker preferred

  • Experience: 2 years related work experience preferably in a nursing home setting
