Assisting Hands Home Care – Merrillville is seeking a compassionate and motivated Social Worker or Community Outreach & Education Manager to join our growing team. In this role, you'll represent our home care agency in the community—connecting with healthcare providers, educating families, and generating referrals through trust and genuine engagement.

This is a relationship-driven role ideal for someone who is passionate about senior care and looking to combine outreach, marketing, and education in a meaningful way.

Responsibilities

Build and maintain strong relationships with hospitals, rehab centers, clinics, discharge planners, senior centers, and other referral sources
  • Conduct educational presentations for professionals and families on topics like dementia care, Parkinson’s care, fall prevention, and aging in place
  • Represent Assisting Hands at health fairs, networking events, and conferences
  • Track outreach activities and leads using our CRM software
  • Schedule and complete weekly visits with referral sources
  • Coordinate with internal staff to ensure quality of service and client satisfaction
  • Participate in strategic growth meetings and occasional on-call rotations
  • Share community feedback and referral trends with leadership team
Qualifications

  • Required: Passion for senior care and community engagement
  • Preferred: LMSW, LBSW, or background as RN, LPN, PT, OT, or in healthcare/social services
  • Excellent verbal communication and presentation skills
  • Organized, proactive, and relationship-driven
  • Proficient with Microsoft Office (especially PowerPoint) or Canva
  • Prior experience in healthcare marketing, home care, or community education a plus
  • Familiarity with CRM systems (or willingness to learn)
  • Reliable transportation and ability to travel locally
