Compensation:

Salary Range: $31.28 - $43.54 / hour

The pay range above represents the lowest possible rate for the position and the highest possible rate. Factors that may be used to determine where newly hired employees will be placed in the pay range include the employee specific skills and qualifications, relevant years of experience and comparison to other employees already in this role. Most often, a newly hired employee will be placed below the midpoint of the range.

Pay for PRN positions is calculated differently than Full-Time, Regular Part-Time, and Part-Time positions. PRN Rates depend on the position and other factors, including years of experience, or shift commitment according to the department policy. PRN differential is added in certain positions.

Responsibilities:

The Clinic LVN Care Coordinator’s primary responsibility is to oversee care management and coordination activities for the patients of the primary care practice. This will include developing and monitoring care management coordination processes and support primary clinical teams with these efforts. It will also include identifying the high acuity patient population and working more directly to ensure care management- care coordination for this patient population. The position may involve some patient triage. The Clinic LVN Care Coordinator will work with the Practice Manager and Medical Director of the practice to develop this position to best serve the needs of the patient panel and the primary care teams.

Qualifications:

Required:

Current California LVN license
Current Basic Life Support (BLS) Certification
High School Diploma or equivalent (GED)

Preferred:

Care Coordination Experience
Medical Office, Clinic or Urgent Care experience
12 months of Nursing Procedure Skills
Knowledge of CPT coding, ICD coding, DNV, State Funded Programs, HMO and Medi-Cal
Bilingual in Spanish
