Job Overview
We are seeking a dedicated and compassionate Home Health Medical Social Worker to join our healthcare team at Signature Healthcare at Home. The ideal candidate will play a vital role in supporting patients and their families through challenging medical situations at home by providing emotional support, resources, and guidance. This position requires a strong understanding of healthcare systems and the ability to work collaboratively with multidisciplinary teams to ensure comprehensive patient care in home health and hospice.
ESSENTIAL JOB FUNCTIONS / RESPONSIBILITIES:
- Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group
- Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required
- Carries out social evaluations and plans intervention based on evaluation findings
- Counsels’ patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties
- Maintains clinical records on all patients referred to social work
- Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs
- Provides information to patients and families/caregivers and community agencies
- Serves as liaison between patients and families/caregivers and community agencies
- Maintains collaborative relationships with organization personnel to support patient care
- Maintains and develops contacts with public and private agencies as resources for patient and personnel
- Participates in the development of the individualized plan of care and attends regularly scheduled interdisciplinary group meetings.
- Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues
- Actively participates in quality assessment performance improvement teams and activities
- Supports mission, values, and goals of Signature Home Health and Hospice
EDUCATION and EXPERIENCE:
- A graduate of a master’s program in social work accredited by the Council on Social Work Education.
- For Social Workers practicing in Oregon, professional license issued by the OR Board of Social Workers (LMSW or LCSW) or CSWA (Clinical Social Work Associate) is required with documentation from the OR Board of Social Work that there is an active Supervision Plan and name of Supervisor.
- Meets qualifications per worked-in state regulations.
- Minimum of one year's experience in health care, hospice experience preferred
Job Types: Part-time, PRN
Pay: $40.33 - $42.54 per hour
