The Hospice Admissions Counselor is responsible for finding appropriate new patients for the Hospice Program.

This positions key functions are:

  • Identifies key referral sources, including appropriate physician offices and puts in place the marketing process in order to help educate the referral source to the services and benefits of the Hospice Program for their terminally ill patients.
  • Presents the advantage of the Medicare Hospice Benefit to referral sources as well as patients and families. This requires a thorough understanding of the Medicare Hospice Benefit, the Medicare Long Term Benefit and the Medicare HMO Benefit.
  • Develops opportunities for and conducts in-service presentations to hospitals and other appropriate referral sources regarding hospice care and the indications for a hospice referral.
  • Understands the importance of follow-up skills to further enhance relationship development and displays good skills as well as creativity.
  • Describes and explains hospice services to potential patient/family and elicits responses to questions regarding patient’s fears, physical limitations and attitude toward hospice care in order to put the patient and family at ease about choosing hospice and to gather pertinent information necessary for admission.
  • Speaks with patient/family regarding financial obligations and insurance coverage, using knowledge of hospice fees as well as potential insurance reimbursement.
  • Confers with Director of Admissions in circumstances of potential admission where reimbursement sources appear not to cover cost of care to ascertain whether the patient may be admitted on a charitable basis.
  • Develops referring relationships with all market sources.
  • Identifies possible contract relationships for the hospice program of care.
  • All signed legal documents are to be fully complete and returned to the Admissions Office within 24 hours of obtaining the signatures.
  • Excellent communication skills, both verbal and written
  • A strong desire to work as an integral member of the admissions/marketing team
  • Some measure of success related to sales and/or marketing
  • This position reports directly to the Director of Admissions.
