SUMMARY
Purpose...............
The Resident Care Coordinator provides direct supervision of care giving within the community. Ensures residents are treated with dignity and respect, recognizing individual needs and encouraging independence. Fosters a homelike atmosphere throughout the community. The Resident Care Coordinator will be the right hand to the Health Service Director, so strong skills as a leader, trainer and support person are required.
Scope..................
This position coordinates care services to residents by the caregivers and medication aides in cooperation with the HSD. Also assists with staff development within the Health Services Department. Willingness to complete other duties as assigned.

DUTIES
Typical................
Provide Care of Residents:
Help with determining resident assessments, service needs. Effectively communicates with residents, family and staff as needed to meet residents’ needs. In addition, help ensure that appropriate documentation has been completed, including charting and ADL documentation.
Managing staff in Health Services Department:
Coordinate departmental schedule and ensure adequate staffing in accordance with company standards, policies, procedures, budgets and state law. Complete staff training, orientation and evaluations. Monitor staffs’ performance including recommendations for pay increases, disciplinary actions, and additional training when required. Assist Facility LN in providing appropriate staff training programs to increase skill levels and meet state requirements. The RCC may be required to work the floor as a caregiver or med aide should staffing not be adequate. Facility may or may not have a Staffing Coordinator to assist with monthly staff schedules and on-call rotations.
Continuous Quality Improvement:
Completes routine audits of MARS. Resident assistant shadowing and training. Updates service plans based on LN/ED assessment. Maintains current and effective service plan binders per state regulations. Complete other audits as assigned by HSD for improvements in department.
Maintaining Compliance with all local, state, and federal guidelines
The Resident Care Coordinator is responsible for following all local, state and federal rules and regulations as they apply to services delivered to all residents. This involves resident care and documentation, and compliance with HIPPA, OHSA, Bloodborne pathogen guidelines.
Safety Compliance:
The Resident Care Coordinator is responsible for ensuring that they work in a safe manner and that they support other staff members in working in a safe manner and that unsafe action are to be corrected. Compliance with Safety Committee Standards is required. Should workplace injuries occur, the staff is to assist the Executive Director and HSD when required with the investigation, return work procedures, and management of the injured worker communication with care provider.
Team Effectiveness:
The Resident Care Coordinator is responsible for fostering teamwork with co-workers, showing initiative in overcoming obstacles, taking actions which assist others beyond job responsibilities, and having a positive impact on co-workers, residents and visitors.

Key Responsibilities:
Monitoring care staff’s job performance
Coaching/Counseling, Evaluating, Communicating
Monitoring staff in administration of medication
Observing residents’ health status, taking actions to address concerns and reporting all significant changes.
Direct supervision of resident care staff.
Cover shifts if need arises, shared on-call for staffing
Must be willing and able to work the floor either as a caregiver or med aide if needed
Schedule & host care conferences as needed and/or required to remain in compliance
Update service plans based on LN/RN/ED assessment.
Communication with resident’s family members, physicians, therapist, etc.
Transcribing/Processing of Physicians orders and communications
Maintain resident charts, MARS & service plan binders
