Substance Abuse Counselor

Summary:

Common Ground is hiring for a LADC, ADC-T, or equivalent, to facilitate groups and individual counseling for our intensive outpatient drug and alcohol treatment center located in Rochester, MN. We offer many different tracks, along the continuum of care, to fit the need of the client.

**This position requires 4 evening groups (4pm-8pm) per week, for group, and the other day is flexible. We are a combined telehealth and in person facility. Work from home options available on Fridays.

Essential duties and responsibilities include the following (other duties may be assigned):

  • Provide counseling, education, and support to clients
  • Utilize counseling skills of the 12 core functions in a group setting
  • Work closely with clients to engage them in the decision-making process and help them understand the benefits of assessment and accessing the appropriate level of care as indicated
  • Documents patient progress through counseling and interaction through groups
  • Creation and monitoring of individualized treatment plans and treatment plan reviews containing client’s goals, activities, and objectives of client care in a timely and professional manner
  • Conduct ongoing evaluation of client needs to make appropriate referrals
  • Makes appropriate amendments in the treatment plan throughout the course of treatment as needed.
  • Directs staff services and interventions per treatment plan.
  • Attend clinical staffing meetings as requested
  • Actively engaging with clients, and developing positive relationships that support the client in meeting their goals
  • Complete documentation as appropriate regarding authorizations, client needs, family needs and agency requests. Document all contacts, incidents, and progress while client is in program
  • Provides concise updates of the client’s progress toward problem resolution to providers as requested or per agreement.
  • Maintain ongoing, current, and required training for best practice service delivery
  • Conduct oneself in a manner that presents a positive reflection on professionalism as an asset to the image of the organization
  • Work collaboratively with all other organizational services and team members
  • Responsible for obtaining and/or maintaining proper licensure/certification according to specific state and program requirements
  • Other duties as assigned.

Requirements:

  • Demonstrated knowledge and skills working with colleagues who thrive in a multi-cultural setting
  • Demonstrated ability and emotional intelligence to work in unpredictable, crisis response, and team-oriented setting
  • Maintain strict and confidential guidelines regarding current and former clients, their families, and referents in accordance with federal and state laws
  • Strong verbal and written communication skills
  • Self-directed and ability to work independently
  • Valid LADC, ADC-T, or equivalent Licensure in the state of MN
  • Familiarity with Procentive software a plus
