EveryMind is hiring a Family Care Manager to join our Linkages to Learning team. This role is responsible for direct care management services within a school and community-based program designed to integrate a system of resources and services for youth and families and to support them in achieving their goals.

Linkages to Learning is a community school partnership that assists students and their families by providing or connecting them to services and resources that address the social, economic, health, and emotional issues that may interfere with student success.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Completes initial intakes/assessments for youth and families referred for services.
  • Collaborates with youth, family and referral source to identify goals to address through the service plan. Evaluates progress and updates service plan at regular intervals.
  • Provides on-going social services to clients which may include securing needed resources, providing necessary referrals and ensuring any court-ordered requirements are met. Assists with completing applications for programs and services that align with service plan goals
  • Maintains compliance with Linkages to Learning contract including:
  • Meeting productivity requirements
  • Maintaining minimum caseload requirements
  • Timely submission of data through County electronic records system
  • Conducts psycho-educational groups for parents and educational workshops as appropriate.
  • Maintains accurate, thorough and timely documentation of client records.
  • Manages funds in assigned budget categories.
  • Assists with seasonal events, such as camp registration, school supply drive and holiday giving program.
  • Provides translation and interpretation services for clients and others as time allows.
  • Assists with orientation and training of new staff.
  • Utilizes regular supervision with Director to evaluate and improve the quality of case management services and ensure that services are culturally appropriate for the client.
  • Maintains client confidentiality according to EveryMind Confidentiality Policy.
  • Adheres to EveryMind and Youth and Family Services Division (YFS) policies established by EveryMind Board of Directors, Chief Executive Officer, Chief Program Officer, and Division Director.
  • Consistently represents EveryMind and the LTL program in a professional manner. Treats all students, families, community members, school personnel, partners, donors, teammates and others with respect.
  • Other duties as assigned.

Qualifications

Education:

  • Bachelor’s degree in social service field required.

Experience:

  • Two years successful experience in the social services field.
  • Experience with case management service delivery strongly preferred.

Skills:

Excellent oral and written communication skills. Must be able to manage multiple tasks while working effectively as a team player. Must have strong communication, listening, coordination, and organizational skills. Provide excellent customer service to those seeking assistance. Strong computer skills with proficiency in Microsoft Office Suite, Zoom, Teams, and other technology platforms. Proficient with data collection and reporting procedures. Bilingual Spanish required.

Abilities:

Effectively interact and work collaboratively in a flexible work environment with a variety of constituents including CEO, board members, clients and family members or caretakers, interns, volunteers, paid staff, and community partners. Ability to express ideas with clarity, confidence and sensitivity. Must be able to maintain confidentiality involving clients in accordance with organizational and federal guidelines.

The employee must be available to perform the duties of the job mostly during core business hours (between 9-5pm, M-F), with flexibility to work evenings or weekends at least 25% of their time. Some flexibility with schedule may be available (evening and/or weekend hours). Employee will work mostly onsite but may work some hours remotely with the approval of supervisor. The employee must pass a criminal background check. Certain job functions may require employee to pass a child sexual abuse and other criminal activity as well as a credit check. All staff providing services to staff or clients, must be free of illegal drugs and abuse of alcohol, and must be fully able to perform all required duties at all times.
