Job Summary
The Director of Social Work is a key leadership position responsible for the development, implementation, and oversight of all social work services within Waveny LifeCare Network’s memory care assisted living facility. This role is integral to ensuring the highest quality of life for our residents by providing comprehensive psychosocial support to them and their families. The Director will ensure all practices adhere to state and federal regulations while fostering an environment of compassion, respect, and open communication to achieve outstanding family satisfaction.
Qualifications:
· Master’s degree in social work from a college/university accredited by The Council of Social Work Education
· Licensed in State of CT (LCSW) - meets all State of Connecticut guidelines for licensed Social Work services
· Minimum of three years Social Work experience. One year of Memory Care Assisted Living experience, preferred.
· Demonstrated experience working with geriatric populations, specifically individuals with Alzheimer's disease, dementia, and other forms of memory impairment.
- In-depth knowledge of the psychosocial aspects of aging and memory loss.
· Demonstrates superior communication skills
· Group facilitation skills
· Demonstrates ability to work as a significant part of an interdisciplinary team
· Ability to demonstrate, teach and supervise
Essential Job Functions/Responsibilities
1. Completes initial psycho-social assessment and re-assessments identifying problems, developing interventions and strategies and setting goals for treatment with and for patient/family, with consideration and respect for patient wishes.
2. Direct and oversee the provision of all social work and psychosocial services to residents, including assessments, care planning, and counseling.
3. Lead the development and implementation of individualized, person-centered care plans that address the social, emotional, and psychological needs of residents with memory impairment.
4. Provide direct counseling and support to residents and their families to help them navigate the challenges associated with memory loss and the transition to assisted living.
5. Act as a resident advocate, ensuring their rights are protected and their voices are heard.
6. Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and all members of the interdisciplinary group.
7. Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary.
8. Works with other team members to understand the social and emotional factors related to health problems and additional stressors in family.
9. Assesses for, and educates interdisciplinary group, on any special needs related to the culture of the patient and family. Includes communication, role of family, space, and any special requirements.
10. Ensures, with other members of the clinical team:
a. the focus of care at time of admission to facility.
b. assists family in understanding changes in the patient’s status related to the progression of end-stage disease.
c. ensures that informed consent, understanding of patient choices for care, and expected outcomes are explained and understood by patient/family.
d. informs patient/family about their rights.
Maintains clinical records on all residents.
