• Job Summary :
• The Social Worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement which include providing care based on all Social Work Practice Act.

Position Qualifications

• Must have a Masters Degree from a Social Work accredited by the Council on Social Work Education
• One (1) year of Social Work experience in a health care setting. Must be able to function in an independent fashion and have considerable maturity of judgement.

• Maintains clinical records on all patients referred to social work;
• Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs;
• Participates in discharge planning;
• Provides information to patients or families/caregivers and community agencies;
• Assists physician and other team members in understanding significant social and emotional factors related to health problems;
• Serves as liaison between patients or families/caregivers and community agencies;
• Maintains collaborative relationships with organization personnel to support patient care;
• Participates in the development of the total plan of care and case conferences as required;
• Maintains and develops contracts with public and private agencies as resources for patient and organization personnel;
• Carries out social evaluations and plans interventions based on evaluation findings;
• Participates in in-service education programs;
• Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse;
• Other duties as delegated by the Clinical Director/Director of Patient Care Services or Clinical Supervisor/Nursing Supervisor;
• Establish accountability for data-driven quality care, service and financial outcomes related to patient transitions of care;
• Other duties as delegated by the Clinical Director/Supervisor.

Job Type: Part-time

Pay: From $70.00 per hour
