Location: Phoenix Transitional Housing Plus
Schedule: Part-Time | Flexible Hours

Are you a passionate and skilled Master’s-level Social Worker looking to make a meaningful impact in the lives of young adults transitioning to independence?

Join our dynamic team at Phoenix Transitional Housing Plus, where we provide support, guidance, and resources to youth (ages 18-21) as they build the foundation for a successful future.

Key Responsibilities:

  • Provide individual case management and support services to AB12 clients
  • Conduct assessments, develop service plans, and monitor progress
  • Facilitate group sessions focused on life skills, self-advocacy, and emotional wellness
  • Collaborate with housing staff, mental health providers, and community resources
  • Maintain documentation in compliance with state and agency standards

Qualifications:

  • Master’s Degree in Social Work (MSW) required
  • Previous experience with transitional age youth or foster care population preferred
  • Strong communication, advocacy, and documentation skills
  • Ability to work independently while being a collaborative team player
