Licensed Social Worker:

* Develop care plans to resolve behavior, mood and psycho-social problems for residents.

* Represents Social Services Department at care planning and family meetings.

* Completes social service notes according to Medicare guidelines.

* Educate resident/legal decision maker about Hospice services and make referral if needed.

* Responds to complaints and other reports and maintain a log.

* Assists with short term rehabilitation admissions process.

Licensed Social Worker Education/Experience

Requires a Master's degree in Social Services plus 2 years long-term care experience; OR a Bachelor's degree in Social Work or related Human Services field plus 5 years long-term care experience.
