Community Health Network (CHN) has been placing experienced nurses, social workers, and physical therapists in home care, hospice, and case management for over 30 years. We offer flexible work options and outside of the box opportunities that focus on your needs and interests.Currently seeking an experienced Medical Social Worker for one our top agency partners to see patients in their homes in Salem, Derry, Newton, Hampstead, NH and surrounding area.
  • Part-time, 20-30 hours/week
  • Flexible to days/hours (make your own schedule!)

As a member of the interdisciplinary team, the Medical Social Worker reports to the Clinical Manager and is responsible for planning and providing supportive services to terminally ill patients and families. Must be comfortable with issues relative to end-of life care, death and dying.Requirements:
  • Current NH LICSW; MSW considered
  • Minimum 1 year of recent Medical Social Worker experience in a Medicare Certified agency.
  • ?Hospice experience preferred.
  • Must be comfortable participating in a collaborative/team environment.
