Are you passionate about supporting residents and their families during some of life’s most meaningful transitions? We are seeking a compassionate, organized, and licensed Social Worker to join our Skilled Nursing Facility team. This role plays a vital part in supporting residents and their families, ensuring psychosocial needs are met, discharge plans are coordinated, and resident rights are upheld. Key Responsibilities:

  • Provide direct social work services to residents in accordance with regulatory requirements

  • Conduct assessments and assist with discharge planning

  • Address resident and family concerns, questions, and care related feedback

  • Develop and maintain a directory of community resources and connect residents to needed services (e.g., Medicare, Medicaid, etc.)

  • Educate staff on residents' rights, including dignity, independence, abuse prevention, and quality of life

  • Support and uphold residents’ rights, and participate in investigations related to abuse or neglect

  • Participate in care plan meetings, interdisciplinary conferences, and utilization review (UR) meetings

  • Complete documentation including MDS assessments, social histories, and progress notes in a timely and accurate manner

  • Assist new residents with orientation, advanced directives, and MDS interviews

  • Coordinate transportation and discharge services as needed

  • Serve as a liaison between families and the care team, ensuring strong communication and support

  • Promote a positive, professional, and compassionate environment for residents and families

  • Ensure compliance with HIPAA and community confidentiality policies


Qualifications:

  • Minimum of a Limited Master’s in Social Work (LMSW) license from the

  • Michigan Board of Social Work (required)

  • Experience working in longterm care, skilled nursing, or a healthcare setting preferred

  • Strong understanding of discharge planning, community resources, and resident rights

  • Excellent interpersonal, organizational, and documentation skills

  • Compassionate, patient focused, and a strong team collaborator
