Position Summary

The MSW Social Worker plays a vital role in supporting the emotional, social, and behavioral health needs of our patients. This position provides psychosocial assessments, care planning, crisis intervention, resource coordination, and advocacy, working closely with a multidisciplinary team to ensure comprehensive care delivery.

Key Responsibilities

• Conduct psychosocial assessments and develop individualized care plans.
• Provide short-term long-term counseling, crisis intervention, and supportive therapy as needed.
• Collaborate with interdisciplinary teams to coordinate services and ensure continuity of care.
• Advocate for patient access to resources such as housing, financial assistance, transportation, and mental health services.
• Maintain accurate and timely documentation in compliance with state, federal, and organizational standards.
• Participate in case conferences, team meetings, and care coordination efforts.
• Educate patients and families about community resources and health-related topics.
• Monitor and follow up on patient progress and evolving needs.
• Maintain confidentiality and adhere to ethical and professional standards of social work practice.

Qualifications

• Master’s degree in Social Work (MSW) from an accredited institution.
• Licensed or license-eligible in the state of Massachusetts (LSW or LICSW preferred).
• At least 1–2 years of experience in a healthcare, community health, or behavioral health setting (experience with geriatric or vulnerable populations is a plus).
• Bilingual in Spanish, Portuguese, or another relevant language preferred.
• Strong interpersonal, communication, and organizational skills.
• Ability to work both independently and as part of a multidisciplinary team.
• Familiarity with electronic health records (EHR) and case documentation.
