Shift: Shift 1/8:30:00 AM to 5:00:00 PM

Minimum to Midpoint Pay Rate:$31.78 - $42.90 / hour

Summary

The Ambulatory RN Care Coordinator utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate, promote and maintain optimal health status of assigned patients. This position functions in a collaborative partnership with the patient, physician and the care team in the office setting. This position serves in an advocacy role to ensure the delivery of quality, efficient, and cost-effective patient-centric health care services. The Ambulatory RN Care Coordinators case load consists of moderate to high risk patients.
