We are seeking a qualified Rehabilitation Counselor interested in joining our Community Residential Rehabilitation (CRR) program at our Delaware County, Pa. location.
The Rehabilitation Counselor provides direct support, counseling and supervision of all assigned residents to ensure a safe, healthy, secure and pleasant environment, which promotes growth and development; to facilitate client growth and development toward independence in self-care. Some of the responsibilities include the following:
Provides and facilitates skill development and support groups which include daily living, socialization, educational and vocational supports and opportunities, and leisure and recreation activities planning.
Provides individual counseling to review, recommend and modify established rehabilitation plan to adapt to special needs and/or service delivery gaps; negotiate and work in tandem with clients to bring together the client and practitioner goals and objectives.
Provides resolution and follow-up to problems, in conjunction with supervisory input.
Maintains a close liaison with the local community and area service providers or agencies to facilitate program cooperation and provide for constructive input and/or alternative resources to current plans.
Establishes an on-going dialogue with other members of the rehabilitation or treatment team, including case-managers, vocational or educational instructors, consultants, members of the medical profession and family members, as needed, to provide continuous supports.
Conducts Intake interviews with applicants to determine appropriate services and provide the Supervisor with recommendations concerning admission.
Associate's degree in the Social Sciences, Healthcare or a related field and 1 year experience in the MH/MR field; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.
Good communication skills, both oral and written.
Ability to work independently and as part of a team.
Some knowledge of mental health system and supports.
Scope:
Frequent talking and listening.
Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with clients.
May require crisis intervention in emergency situations.
Must be available for local travel and possess a valid driver's license with an acceptable driving record.
May be required to report to work during emergencies including inclement weather.
