Your Role:

• Coordinate the intake process for pediatric hospice patients, ensuring all documentation is complete and accurate
• Work with healthcare providers, referral sources, and families to ensure a smooth and compassionate admission process
• Maintain clear communication with the clinical team to ensure timely care for new patients
• Verify eligibility and insurance information, answering any questions patients or families may have

What You Bring:

• Experience in healthcare intake, hospice, or home health is preferred
• Strong organizational skills and attention to detail
• Compassionate and empathetic approach to patient care
• Excellent communication and customer service skills
• Spanish Speaking
