Summary

We are seeking a Licensed Clinical Social Worker to join our team at Advanced Comfort Health. In this role, you will play a crucial part in supporting patients through their healing journeys by providing emotional support, counseling, and therapeutic interventions. Your expertise will be vital in enhancing the overall quality of care we deliver.

Responsibilities

• Conduct comprehensive patient assessments to identify emotional and psychological needs.
• Provide individual and group therapy sessions focused on coping strategies and emotional support.
• Utilize cognitive behavioral therapy and motivational interviewing techniques to facilitate patient progress.
• Collaborate with medical staff to develop integrated care plans that address both physical and mental health needs.
• Support patients and their families through education about wound care and recovery processes.
• Document patient progress and treatment outcomes in accordance with healthcare regulations.

Requirements

• Master's degree in Social Work from an accredited institution.
• Current licensure as a Clinical Social Worker (LCSW) in the state of practice (Minnesota).
• Experience in healthcare settings, particularly in skilled nursing or assisted living, is preferred.
• Strong skills in cognitive behavioral therapy, motivational interviewing, and patient assessment.
• Excellent communication skills with a compassionate approach to patient care.
