Job Title: Clinical Social Worker
Assignment Length: 13 weeks (Can go long term)
Location: Canton, MA
Shift: 8-4PM or 9-5PM, or a mix of both.
Pay Rate: Up to $35/hour
Job Summary:
The Clinical Social Worker coordinates, monitors, develops and implements comprehensive treatment plans for physically disabled patients. The Clinical Social Worker review treatment outcomes and provides communication with state and local agencies to ensure program implementation. Supervises Clinical Social Worker Interns. Assesses competency levels of patients and provides patient and family therapy. Provides other job-related duties as assigned by supervisor.
Duties and Responsibilities:
- Assesses competency levels of pediatric patients (18 to 22 years old) to determine need for guardianship.
- Interviews patients and families at a time of pre-admission evaluation to assess appropriateness for admission. Documents findings and makes recommendation to the Admission Committee.
- Meets with family of each new patient, obtain a complete psycho-social history which is placed in patient’s medical record.
- Coordinates and monitors assigned program activities in order to ensure effective operations and compliance to established standards.
- Reviews and analyzes data on a continuous basis through the care planning process to determine progress by measuring outcomes to make recommendations for changes in procedures affecting guidelines, interventions and approaches that accomplish program/treatment objectives.
- Conducts group therapy with patients to assist them in understanding and developing coping mechanisms regarding their physical disabilities and relationship issues.
- Monitors and evaluates the progress of patients receiving services through on site visits, discussion with service providers, review of progress of progress reports, etc. in order to determine if placements are appropriately meeting the patient’s needs.
- Maintains and liaison with various public and private agencies, organizations and officials in other to exchange information and/or to develop or improve services and placement opportunities for patients.
- Confers with agency staff members in order to obtain and exchange information concerning clients, agency services and to resolve problems.
- Makes determinations of suspected, alleged or reported violations of agency standards, regulations etc. and recommends appropriate actions
- Perform other job duties as needed or assigned by the client.
Job Requirements:
- At least 1 year of experience delivering psychological services to children, adolescents, and young adults with intellectual disabilities.
- Active MA LCSW License
- Fully Vaccinated against Covid-19
