About Job
At Crossroads Hospice & Palliative Care, we believe end-of-life care is about more than clinical treatment—it’s about helping people feel seen, heard, and supported. We walk alongside patients and families during one of life’s most profound transitions, offering guidance, dignity, and comfort every step of the way.
As a Social Worker, you'll be supported by experienced mentors as you learn how to guide conversations about serious illness, help families process complex emotions, and connect them with the resources they need. You’ll be part of a collaborative team—including nurses, aides, chaplains, and physicians—where your voice matters and your contributions make a real difference.
