About Job
The Social Worker (MSW) provides care management and social support to PACE program participants to promote independence and well-being.
- Offers services including assessments, case management, counseling, referrals, and care planning.
- Acts as a liaison between participants, families, and interdisciplinary teams, ensuring coordinated care.
- Conducts home visits, facilitates group activities, and advocates for participants’ needs.
- Maintains accurate documentation and participates in program development and quality improvement.
- Requires a Master’s in Social Work, state licensure, and relevant experience, preferably with elderly or frail populations.
- Offers competitive salary ($62K-$68K) and benefits such as medical/dental, 401(k), paid time off, tuition reimbursement, and more.
- Schedule: Monday–Friday, 8:30 am–4:30 pm, with on-the-road work.
