The Behavioral Health Care Manager is a core member of the collaborative care team, including the patient’s primary care provider and psychiatric consultant, as well as the larger primary care team or medical team. The Behavioral Health Care Manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, other mental health providers. Responsibilities include providing therapy and case management, collaborating closely with primary care providers, screening and assessing patients using tools such as PHQ-9 and GAD-7, monitoring treatment progress, supporting medication management, providing brief evidence-based behavioral interventions, facilitating referrals, maintaining documentation in the EHR and registry, developing relapse prevention and discharge plans, ensuring safety and compliance with policies, promoting professional development, and performing other duties as assigned. The position requires bilingual proficiency in Spanish and English, graduation from an accredited Mental Health Master’s Program, and current/active licensure as an independent licensed mental health professional in Texas (LCSW preferred, LMSW considered with supervision hours, LPC, LMFT, or Licensed Psychologist).
