Job Summary

Looking for a dedicated and compassionate Masters Social Worker (MSW) to join our PACE (Program of All-Inclusive Care for the Elderly) team in El Cajon, CA. This role provides an excellent opportunity to make a lasting difference in the lives of senior patients. The MSW will provide social services support to the elderly population in an outpatient clinic setting, including occasional in-home visits.

Title: Masters Social Worker (MSW)/Social Worker
Location: El Cajon, CA
Schedule: Monday – Friday, 8:00 AM – 5:00 PM (On-site with occasional home visits)
Compensation: $37.05 - $47.80 per hour + Benefits

Key Responsibilities

• Provide social work services to patients in the PACE outpatient clinic and in-home settings as needed
• Conduct assessments, develop care plans, and support patients with emotional, social, and resource needs
• Collaborate with interdisciplinary care teams including medical staff, therapists, and caregivers
• Educate patients and families on community resources and care options
• Maintain accurate records and documentation

Qualifications Required:

• Master’s Degree from an accredited School of Social Work
• Valid California driver’s license and access to reliable, insured transportation
• Strong commitment to working with the senior population

Preferred:

• At least 1 year of experience in a clinical healthcare setting
• Previous experience as a Clinical Social Worker
• Experience working with frail or elderly patients

Language:

• Bilingual English and Arabic highly desired
• Spanish also helpful
