- This social worker serves as a valued member of the interdisciplinary genetics team, providing comprehensive, patient-centered care to pediatric and adult patients with rare and complex genetic and metabolic disorders, which includes conducting psychosocial assessments that address factors impacting patient and family well-being, treatment adherence, and health outcomes; delivering individualized clinical interventions that include supportive counseling, adjustment-to-diagnosis, care coordination, and crisis intervention; adhering to ethical, organizational and professional standards; and maintaining timely, accurate, and comprehensive documentation in the electronic health record to support care coordination, interdisciplinary collaboration, and continuous quality improvement.

Job Description

- Functions as a member of the interdisciplinary team and provides Clinical Medical Social Work services to patients and their families to improve or maintain social, emotional, functional and physical health.
- Provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services.
