Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Social Determinants of Health (SDOH) team!The Care Coordinator assists patients onsite at the clinic and collaborates closely with the clinical care team to connect Erie patients with both internal and community resources, aimed at enhancing their overall health and well-being.

Main Duties & Responsibilities

  • Screen patients for needs around SDOH and provide linkage to services such as emergency financial assistance, transportation, food assistance, etc.
  • Enroll patients in services, benefits, and programs for which they are eligible (pharmaceutical programs, LINK, financial applications for hospital referrals, etc.)
  • Educate and empower patients to navigate future SDOH needs
  • Distribute and track SDOH resources including Top Box Food distribution, ordering Uber Health rides, and distributing other material resources
  • Use Epic to document work, respond to referrals and create SDOH panels and outreach risks
  • Build effective relationships with local social service agencies and advocate for clients in the process.
  • Performs other duties as assigned.

Qualifications

QUALIFICATIONS

Education:

  • High School diploma or equivalent required
  • Associates degree or Bachelor’s degree preferred

Experience:

  • A minimum two years of work experience in community health, social or community services, or comparable experience preferred.

Skills and Knowledge :

  • Verbal and written communication skills, including high level of comfort presenting to groups.
  • Intermediate proficiency in Microsoft Office preferred
  • Local travel is required for the position.
