Location: Oak Park, IL
Status: Full-Time | Monday–Friday Schedule
Reports To: General Manager

You’ll work in a supportive and empowering environment where your voice and leadership matter.

- What You’ll Do

  • Conduct in-home consultations with prospective clients and families
  • Create and manage personalized Care Plans
  • Build and maintain strong client relationships
  • Conduct timely quality assurance visits (24 hours, 7 days, 30 days, quarterly)
  • Serve as the go-to for client/family concerns — you’ll listen, solve, and care
  • Collaborate with scheduling to ensure excellent caregiver matches
  • Support caregiver performance with recruitment and training teams
  • Maintain accurate records in WellSky and client files
  • Participate in rotating on-call leadership support

- What You Bring

  • Bachelor’s degree in healthcare, social work, or human services (preferred)
  • 2+ years in client care, case management, or home care leadership
  • Excellent communication, organizational, and critical thinking skills
  • Tech-savvy: Microsoft Office, CRM/EMR systems like WellSky
  • Valid driver’s license, reliable transportation, and insurance

Job Type: Full-time

Pay: $50,000.00 - $60,000.00 per year
