Resident Care Coordinator
POSITION SUMMARY
The Resident Care Coordinator (RCC) helps create a joyful and vibrant community by participating in the orientation and training of staff as assigned by community leadership and serving as a mentor and resource to team members.
ESSENTIAL JOB FUNCTIONS

  • Makes and keeps care staff schedule updated as well as finds coverage to fill PTO requests. Reviews assignments with management team.
  • Acts as on-call staff and fills in shifts when requested by the Director of Nursing Services.
  • Reads staff communication records and maintains familiarity with resident records as required.
  • Assists residents with activities of daily living, assistance with medications, treatments and other care while encouraging self-care and independence.
  • Provides resident services as indicated on the resident’s assessment and service plan. This includes bathing, grooming, medication, and providing assistance or administrative duties related to daily activities/living.
  • Observes problems, concerns, issues with employees and/or residents and communicates them appropriately to Director of Nursing Services.
  • Records resident information as required.
  • Assists with the orientation of new staff members to job responsibilities. Acts as a lead/trainer for care staff.
  • Monitors for receipt of physician orders on a daily basis. Follows up with physicians on orders not returned.
  • Demonstrates knowledge of and adheres to procedures for fire, life safety, disaster, security work, and other emergency procedures.
  • Notifies physician, nurse and / or designated agent(s) of resident status/change as appropriate.
  • Coordinates care with outside agencies.
  • Participates in service plan meetings with residents and families.
  • Ensures all medications are placed in a timely manner to residents do not run out of medications.
  • Ensures all doctor’s orders have been transcribed correctly in the MAR (Medication Assistance Record) and ensures all orders have been signed off by the Medication Aide.
  • Completes monthly MAR reviews and follows up on any discrepancies.
  • Attends and participates in required meetings and trainings. Stays current on system assigned tasks and duties.
  • Performs other tasks/duties as assigned.
EXPERIENCE, QUALIFICATIONS & SKILLS
  • LPN, CNA, or HCA license
  • Previous experience in Assisted Living
  • A high school diploma or equivalent.
  • Ability to read, write, speak and understand English.
  • Meets all mandatory health and training requirements per State regulation.
  • Maintains a current First Aid and CPR certification.
  • Pass a criminal background and fingerprint check.
  • Demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
  • Has compassion and a special interest in working with the senior population.
  • Ability to work a flexible schedule, including weekends, holidays and overtime, to meet the requirements of the position.
  • Ability to relate positively and communicate appropriately with residents, families, community members, volunteers and other employees.
  • Ability to creatively problem-solve in resident care
  • Maintains clean, neat, comfortable, safe environment for residents, staff, and visitors.
  • Ability to work independently (with minimum supervision)
  • Maintains current Food Handlers Card.

The employee must occasionally lift and/or move residents. Lifting 25-50 pounds occasionally. Reaching below shoulder height frequently. Kneel; stoop and squat are required occasionally. Standing long periods of time.
