As a Community Liaison, you will liaise between Jennicare Home Health Services and hospital personnel, patients, physicians, community organizations, retirement communities, assisted living, and skilled nursing facilities.
Duties And Responsibilities Include
- Maintaining ongoing communication with case managers, discharge planners, and physician office staff.
- Occasionally attend hospital case manager/discharge planner meetings and discuss the agency's concerns, and also provide information related to changes in home health care guidelines
- Compiling information needed to establish definitive patient home care plans, including assessing the appropriateness of requested services, and providing such information to the clinical staff prior to patients' admission for home health services.
- As the Community Liaison, you are required to have an outgoing and engaging personality with the ability to quickly build rapport and develop working relationships with referral sources.
- Assist in the coordination of care between agency patients and staff to ensure agency clients are provided services as required.
Responsibilities include:
- Answer patients' inquiries, take referrals, and schedule home care visits.
- Schedule and interview, and orient newly hired staff.
- Develop strong and lasting relationships with referral sources and community resources.
- Create, maintain, and continually update patients' and staff profiles, including documentation of patients' staffing needs based on established treatment plans.
- Work closely with the Administrator and other professional staff to discuss scheduling activities.
- Communicates regularly with staff through verbal, written, and individual meetings about their schedules and takes necessary action to accommodate staff schedule needs as necessary.
- Occasionally visit primary physician offices to obtain necessary documentation, such as physician signatures on orders when requested by the Administrator and Medical Secretary, to comply with the regulatory bodies’ requirements.
- Participate in patient care by visiting patients’ homes to provide care when needed.
- Other duties as assigned.
- Education in Business or a Health-related field preferred, but not necessary. Required on-the-job training will be provided as necessary.
