WHAT YOU CAN EXPECT

  • Describes the PACE program, either by telephone, electronically, or in-person to interested persons including candidates for enrollment, caregivers, or others in the community interested in the PACE program.

  • Verifies the eligibility of program applicants as per current program eligibility as stated in state and federal regulations.

  • Collaborates with other departments within the hospital to align interdepartmental functioning, goals, and expectations as outlined in the SLA.

  • Receives and generate referrals from agency staff and community sources; follow up with all potential participants and referrals regarding program enrollment.

QUALIFICATIONS

  • Bachelor's Degree - Required

  • Bachelor's Degree- Social Work- Preferred

  • Master's Degree- Social Work- Preferred

  • 1 Year Social Services - Required

  • 1 Year working with Frail or elderly - Required

TRAVEL IS REQUIRED:

Up to 50%
