About the Transitions Opioid Treatment Program

Transitions provides comprehensive, evidence-based medication-assisted treatment (MAT) for individuals living with opioid use disorder. Our program combines FDA-approved medications—such as methadone and buprenorphine—with a full range of wraparound services, including individual counseling, group therapy, case management, mental health support, and nursing care.

Our mission is to meet each client where they are in their recovery journey. We aim to prevent overdose, ease withdrawal symptoms, stabilize brain chemistry, reduce cravings, and support clients in achieving their personal goals and building a healthier, more fulfilling life.

What You’ll Do

  • Maintain and interact with a caseload of clients with diverse characteristics either in-person and in some cases, virtually.
  • Evaluate client’s mental health condition based on review of client information, including bio-psycho-social assessments, psychological evaluation and diagnostics. Collecting information about the client through interviews, observations, and tests. Prepares and maintains treatment records and reports.
  • Develop and implements treatment plans based on clinical experience, knowledge, and understanding of the client’s information and history. Acts as a client advocate to coordinate required services or to resolve emergency problems in crisis situations.
  • Counsel clients, individually and in group sessions specific to the program format, to assist in overcoming dependencies, adjusting to life, and making changes. Maintains confidentiality of records relating to clients treatment.
  • Work within an interdisciplinary team to develop treatment plans for client(s) being served.

Requirements of the Position

  • Minimum of a High School Diploma and be certified or registered as a Substance Use Disorder Professional by the Missouri Credentialing Board, Inc. as either a CADC, CRADC, or MAADC II is required.
  • Preference given to those with a Master’s Degree in a social science field with a provisional or clinical license in the State of Missouri.
  • Flexibility and/or availability to work a consistent schedule of 6:30 am – 3:00 pm Monday – Friday.
  • Working knowledge of mental illness and substance use disorders.
  • Excellent customer service skills with a recovery focus.
  • Quality written, oral and interpersonal communication skills.
  • Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as G-Suite.
