Summit Home Care and Hospice is searching for an experienced and motivated Medical Social Worker to join our home health and hospice teams. Hospice & Home Health Medical Social Worker

  • Shift: Monday – Friday; Standard Business Hours
  • Office Location: 800 Yard St. Suite 300 Columbus, OH 43212
  • Work Setting: Hybrid & Some Travel Required
  • Service Areas: Franklin County & Surrounding Counties
  • Travel: Some Required (Mileage Reimbursement, $0.48/Mile)
  • Salary Range: $68,000 - $72,000 per Year

Job Summary

  • The Home Health and Hospice Medical Social Worker (MSW) is responsible for providing a wide range of psychosocial services to terminally ill patients and their families, as well as patients receiving home health care.
  • This includes assessing psychosocial status, implementing care plans, and ensuring compliance with state Medical Social Work Practice Acts and company policies.
  • This is a cross-functional/hybrid role that will primarily support Home Health team but may also support the Hospice team when needed.

Responsibilities

Psychosocial Assessment and Care

  • Conduct initial and ongoing psychosocial assessments of patients and families/caregivers.
  • Develop and implement care plans including social service goals, supportive counseling, problem-solving, and community referrals.
  • Provide pre-bereavement and bereavement support, including support groups as appropriate.

Patient and Family Support

  • Serve as an advocate and support person for patients and families/caregivers.
  • Provide information and refer to additional services as needed regarding practical and environmental needs.
  • Assist families with advanced directives, end of life planning, and care facility placements when appropriate.
  • Participate in discharge planning and ensure continuity of care.
  • Link patients with community resources and services.

Collaboration and Communication

  • Serve as an active participant with the Hospice IDG and attend routine IDG meetings every 15 days as scheduled.
  • Maintain collaborative relationships with interdisciplinary team members.
  • Participate in team meetings, case conferences, and in-services to contribute to patient care planning.
  • Serve as a liaison between patients/families and community agencies, maintaining and developing contracts with public and private resources.

Documentation and Compliance

  • Maintain accurate clinical records, progress notes, and reports in compliance with local, state, and federal laws.
  • Ensure compliance with company policies, Medicare regulations, and professional standards.
  • Manage related expenditures in a fiscally responsible manner.
  • Perform other duties as delegated by the Hospice Clinical Manager/DON/Administrator/Hospice and Home Health MSW.

Qualifications

Qualifications:

  • Master's degree in social work (MSW) from an accredited school required.
  • Licensure and/or certification as required by the state.
  • Minimum of two years of social service experience in healthcare setting or social services.
  • Knowledge of community resources and ability to obtain them for patients and families.
  • Understanding of hospice philosophy and principles of death/dying.
  • Ability to work independently in an interdisciplinary setting.
  • Valid driver’s license and an insured automobile in good working order.
  • Excellent observation, judgment, and communication skills, with the ability to provide timely written documentation.

  • Ability to lift, pull, push, and manipulate equipment and patients/clients.
  • Ability to sit and/or stand for long periods.
  • Ability to operate an automobile and provide services in patients’ homes.
  • Provide and maintain a safe environment for patients.
