Contract LCSW – Palliative Care (Denver Metro Area)

We’re seeking a Licensed Clinical Social Worker (LCSW) to provide therapy and emotional support to patients and caregivers across the Denver metro area. This is a 1099 contractor role designed for someone who values flexible scheduling, purposeful work, and strong clinical autonomy within a supportive team.

What You'll Do

  • Provide in-person therapy visits to patients receiving palliative care in their homes, assisted living facilities, or skilled nursing facilities across the Denver area.
  • Deliver telehealth support as appropriate to ensure timely follow-up and caregiver connection.
  • Conduct psychosocial assessments, support goals of care conversations, and provide counseling for patients and families navigating serious illness.
  • Assist with resource navigation, caregiver support, and emotional processing during complex medical decision-making.
  • Collaborate with Journey of Care’s nurse practitioners, RNs, and care coordinators to deliver cohesive, patient-centered care.
  • Document visits in our electronic health record and communicate updates to the team.

This Role is Ideal for You If You...

  • Are a Licensed Clinical Social Worker (LCSW) in Colorado with at least 2 years of experience in palliative care, hospice, geriatrics, or medical social work, or grief counseling
  • Live in or near the Denver metro area and are able to travel to patient visits locally
  • Are comfortable working autonomously in community-based settings
  • Enjoy providing emotional support, guiding families through difficult transitions, and facilitating meaningful goals-of-care discussions
  • Are interested in value-based care models that prioritize quality and alignment with patient values
  • Thrive in a collaborative, purpose-driven care team

Position Overview

Pay: $40.00 - $45.00 per hour

Expected hours: 2 – 20 per week
