How you’ll contribute

Are you passionate about guiding patients through their rehabilitation journey with clarity, care, and collaboration? We’re looking for a dedicated Case Manager to be a vital part of our interdisciplinary team, ensuring seamless care coordination and successful discharge planning.

How you’ll contribute

• Complete orientation and ongoing competencies to support departmental excellence
• Collaborate across departments to resolve barriers to care or discharge
• Keep documentation on track—including the Individual Plan of Care (IPoC)—in line with CMS guidelines
• Schedule and lead meaningful family conferences to keep caregivers informed
• Collect and report performance improvement data to elevate care quality
• Your Expertise:
• Strong organizational and interpersonal skills
• Knowledge of the rehabilitation process and age-appropriate care planning

Qualifications and requirements:

• Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist.
• Certification in Case Management or Rehabilitation Nursing preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC
• Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
