Position Summary:

The Health Homes Care Coordinator – Allied Staff is responsible for providing person-centered care coordination and service linkage for DSNP and Medicaid-enrolled individuals with chronic conditions and complex care needs. The role focuses on supporting individuals to access medical, behavioral health, and social services to improve overall health outcomes. This position does not require clinical licensure but must meet the allied staff qualifications per program guidelines. This position requires a strong understanding of medical terminology and experience in both medical and dental office environments. The allied staff will be responsible for coordinating patient care, managing office tasks, and facilitating communication between clients and care coordinators and healthcare providers.

Responsibilities

  • Conduct targeted outreach to locate and engage individuals referred to the Health Homes program, including those considered hard to reach or lost to follow-up.
  • Build rapport and trust with clients to facilitate enrollment and service acceptance.
  • Educate potential members and their families about the Health Homes program, its benefits, and the care coordination process.
  • Complete outreach documentation, including contact attempts, outcomes, and barriers to engagement.
  • Collaborate with hospitals, clinics, shelters, behavioral health providers, and community-based organizations to locate clients.
  • Assist in gathering intake documents and verifying eligibility (e.g., Medicaid status, chronic conditions).
  • Coordinate and attend initial appointments with care managers, healthcare providers, or other service agencies as needed.
  • Support the development of initial assessments and care plans by gathering basic social history, strengths, and barriers.
  • Maintain confidentiality and uphold HIPAA regulations at all times.

Qualifications

  • Previous experience in medical administrative support or a similar role is preferred.
  • Familiarity with medical terminology is essential for effective communication within the healthcare environment.
  • High school diploma or associate degree in a related field (e.g., Human Services, Community Health, Social Work)

(Bachelor’s degree preferred, but not always required for allied staff roles)

  • At least 1 year of experience in community outreach, peer support, case management, or health navigation.
  • Ability to work in diverse neighborhoods and with individuals who may be homeless, unstably housed, or have behavioral health needs.
  • Strong communication and interpersonal skills.
  • Ability to document clearly and follow protocols for client tracking.
  • Reliable transportation and valid driver’s license (if outreach is field-based).
  • Proficiency in office software applications to manage scheduling and documentation efficiently.
  • A commitment to maintaining confidentiality and adhering to HIPAA regulations.

Schedule:

  • 8 hour shift
