???? We're Hiring: Full-Time Care Manager (Wilmington, NC Area) 

???? New Hanover & Brunswick Counties | On-Site + Community-Based | Full-Time 

Assistance for Life Care Management Services (AFL) is seeking a compassionate, skilled, and self-directed Care Manager to join our growing team. Our mission is to support older adults and their families by navigating complex care situations and improving quality of life through expert guidance, advocacy, and coordination. 

 


???? About the Role 

As a Care Manager, you’ll work directly with older adults and their families to assess needs, create personalized care plans, coordinate resources, and provide ongoing support including crisis management. You’ll serve as the central point of communication between the client, family, healthcare and other service providers to ensure the client receives optimal care tailored to their unique situation. 


This is a full-time position based in New Hanover County, NC as well as parts of Brunswick and Pender Counties.  


The world of care management is different each day.  The person most successful in this role will thrive on flexibility, creating your own schedule, and being adept at self-organization. Time management, ability to multi-task, and effective communication skills are a must.   A proactive approach to helping clients navigate the complicated landscape of aging is one of the most crucial aspects of care management. 



✅ What You'll Do 

  • Participate in comprehensive assessments of client health, home safety, support systems, and long-term planning needs 
  • Develop and maintain individualized Plans of Care 
  • Coordinate services such as home care, medical appointments, relocation support, and household management 
  • Act as liaison and advocate between clients, families, and service providers 
  • Support clients during medical crises and care transitions 
  • Maintain detailed documentation and participate in team case reviews 
  • Provide on-call coverage on a rotating schedule (includes some weekends/holidays) 

 


????‍⚕️ Qualifications 

  • Education: 
  • Preferred: Master’s in Social Work, Nursing, Occupational Therapy, Gerontology, or related field 
  • Accepted: Bachelor’s in Social Work, Nursing, Gerontology, Psychology, Health & Human Services or related 
  • Experience: 
  • Preferred:  Minimum 2 years in care management and/or geriatrics 
  • Other Requirements: 
  • Strong interpersonal and communication skills 
  • Excellent organizational and problem-solving abilities 
  • Confident in using and managing a variety of software platforms and devices; able to support clients with basic technology needs 
  • Knowledge of aging-related services and local resources 
  • Reliable transportation, valid driver’s license, current auto insurance 
  • Must reside in New Hanover, Brunswick or Pender County
