Position Summary
- Provides social work support based on initial and ongoing assessment of the patient and family needs. Counseling services are provided to assist the patient and family in minimizing the stress and problems that arise from the terminal illness, related conditions, and the dying process.
Primary Position Responsibilities
- Provides social work care to patients and families of Hunterdon Hospice through participation in the admission process
- Provides counseling for patients and families to assist in minimizing the stress and problems that arise from the terminal illness, related conditions and the dying process
- Assesses Bereavement risk and creates a plan of care based on individual needs of family and initiates plan of care at time of patient death
- Actively participates in Hunterdon Hospice Quality Improvement

- Hunterdon Hospice IDT, Visiting Health and Supportive Services staff, Hunterdon Medical Center staff, Hunterdon Healthcare Physicians and physicians outside the Hunterdon Healthcare system, Internal and External Hunterdon Healthcare community

Qualifications
- Minimum Education:
- Required:
- Master's Degree in Social Work
- Preferred:
- None

- Minimum Years of Experience (Amount, Type and Variation):
- Required:
- Minimum of one year experience post graduate degree conferral in a health care setting
- Preferred:
- Two years of experience in hospice or palliative care
- Required:
- License, Registry or Certification:

- New Jersey LSW licensure
- Preferred:
- New Jersey LCSW
- Required:
- Knowledge, Skills and/or Abilities:
- Required:
- Loss / grief issues, bereavement counseling, working with terminally / chronically ill patients
