Job Description:
Ascend Living, a residential senior care home in Humble, Texas, is seeking a motivated and connected Community Liaison to help expand our resident population through referral partnerships and strategic outreach.

In this role, you’ll represent Ascend Living within Community Health Networks, senior services, and other referral sources. You'll serve as the link between families, providers, and our admissions team—helping coordinate smooth transitions into our home.

Key Responsibilities:

• Promote and represent Ascend Living within Community Health Networks, hospitals, rehab centers, churches, home health agencies, and other community-based organizations
• Secure resident placements and referral opportunities
• Coordinate move-ins with the admissions team to ensure a seamless onboarding experience
• Build and maintain strong relationships with families and referral sources
• Schedule and facilitate home tours as needed

Compensation:

• Commission-based: Earn one full month’s rent per successful placement
• Paid in two installments during the 2nd and 3rd months of residency
• Bonus opportunities available for strong performance and volume

Requirements:

• Must have an established book of business or referral network
• Prior experience in placement, liaison work, or community outreach within healthcare, home health, or senior living settings
• Excellent interpersonal and communication skills
• Strong organizational skills and the ability to manage multiple placements
• Familiarity with senior care housing models such as assisted living or group homes is preferred
