About The Role

We are looking for an MMA (Managed Medical Assistance) Care Manager. Independent Living Systems (ILS) together with its affiliated health plans, Florida Community Care and Florida Complete Care, are committed to improving the quality of life for the most vulnerable populations. Through our commitment to comprehensive care, we strive to maximize autonomy and improve overall well-being for those we serve. As an MMA Care Manager, you will coordinate a comprehensive range of care activities for the members on your caseload, ensuring the efficient use of resources and facilitating access to the care required to enhance their quality of life. You will also conduct health risk assessments to identify needs and gaps in care, collaborating with members to develop personalized care plans that address these needs and support them in achieving their health goals.

Minimum Qualifications

  • Bachelor's degree in social work, sociology, psychology or a related field
  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed to practice in the state of Florida
  • Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities and or disabilities in a case manager or similar role or,
  • Minimum of 2 years of relevant experience working with medically complex children
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Bilingual in English and Spanish is preferred
  • Relevant experience may substitute for the educational requirement on a year-for-year basis

Preferred Qualifications

  • Master's degree in nursing, social work, or a related field
  • Certification in care management
  • OB experience preferred
  • Experience working with Adult, elderly or disabled populations
  • Bilingual in English and Spanish

Responsibilities

  • Conduct assessments of clients' medical needs and develop individualized care plans
  • Collaborate with medical professionals to ensure that clients receive appropriate medical care
  • Monitor clients' progress and adjust care plans as needed
  • Provide education and support to clients and their families
  • Maintain accurate and up-to-date records of clients' care and progress

Skills

As an MMA Care Manager, you will leverage your strong communication and interpersonal skills to build meaningful relationships with members and their families. Drawing on your clinical and professional expertise and critical thinking abilities, you will develop and implement personalized care plans. Your organizational and time management skills will be essential in managing a caseload of members while ensuring accurate record-keeping. Additionally, your capacity to work both independently and collaboratively as part of a team will be key to delivering high-quality care to our members
