Intensive Care Coordinator
In Holyoke, MA, as part of the Children’s Behavioral Health Initiative (CBHI) the Care Coordinator facilitates the process of Intensive Care Coordination (ICC) services. Complete youth assessments that determine strengths, needs, and eligibility for the service. Further develops and facilitates Care Planning Team meetings as well as risk management/safety plans when appropriate. Coordinate all team member information and meetings, necessary updates that ensure a unified approach in an effort to achieve the family prioritized vision and goal(s). Be comfortable with public speaking, be strengths based and be able to identify the individual and cultural issues relevant to understanding and supporting the needs of the youth and family.
