The Care Coordinator plays a vital role in ensuring that patients receive comprehensive and coordinated care throughout their healthcare journey. Duties

- Administer medications as prescribed and monitor patients for any adverse reactions.
- Utilize electronic health record systems such as Epic to document patient information accurately.
- Maintain up-to-date knowledge of ICD-10 coding for accurate billing and documentation.
- Provide education to patients and families regarding treatment plans, medications, and follow-up care.
- Assist in occupational health assessments and ensure compliance with health regulations.
- Support patients during procedures, including basic life support when necessary.
- Facilitate communication between nursing homes, ICU units, and other healthcare settings to ensure continuity of care.
Skills
Candidates should possess the following skills:
- Proficiency in Basic Life Support techniques.
- Familiarity with performing spinal taps as part of patient care.
- Experience in medication administration within a clinical setting.
- Knowledge of Epic electronic health record systems for efficient documentation.
- Understanding of ICD-10 coding for accurate medical billing.
- Experience working in nursing home environments or intensive care units (ICU).
- Strong skills in patient assessment to identify needs effectively.
- Proficient understanding of medical terminology to communicate clearly with healthcare teams.
