Overview
The Care Coordinator plays a vital role in ensuring that patients receive comprehensive and coordinated care throughout their healthcare journey. Duties

• Provide education to patients and families regarding treatment plans, medications, and follow-up care.
• Assist in occupational health assessments and ensure compliance with health regulations.
• Support patients during procedures, including basic life support when necessary.
• Facilitate communication between nursing homes, ICU units, and other healthcare settings to ensure continuity of care.

Skills

• Familiarity with performing spinal taps as part of patient care.
