Overview
We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. This role is essential in providing support and resources to patients and their families, particularly in outpatient clinic settings, nursing homes, and geriatric care environments. The Medical Social Worker will work collaboratively with healthcare professionals to address the psychosocial aspects of patient care, ensuring that individuals receive comprehensive support tailored to their needs.

Responsibilities

• Conduct assessments of patients’ social, emotional, and environmental needs to develop effective care plans.
• Provide counseling and support to patients and their families regarding health-related issues, including coping strategies for chronic illnesses.
• Collaborate with interdisciplinary teams to coordinate care and ensure holistic treatment approaches.
• Facilitate access to community resources and services that can assist patients in managing their health conditions.
• Maintain accurate medical documentation in accordance with ICD-10 standards and organizational policies.
• Implement cognitive behavioral therapy techniques as appropriate to help patients manage behavioral health challenges.
• Advocate for patients’ rights and needs within the healthcare system.
• Participate in discharge planning processes to ensure continuity of care post-treatment.

Skills

• Strong background in social work with a focus on outpatient settings, geriatrics, or nursing home environments.
• Proficiency in medical documentation practices and familiarity with ICD-10 coding.
• Experience with cognitive behavioral therapy techniques for effective patient engagement.
• Knowledge of behavioral health issues and the ability to provide appropriate interventions.
• Excellent communication skills, both verbal and written, for effective interaction with patients, families, and healthcare teams.
• Empathy and strong interpersonal skills to build rapport with diverse patient populations.
• Ability to work independently as well as part of a collaborative team environment.

Job Types: Part-time, Contract

Pay: $28.17 - $33.92 per hour

Expected hours: 5 – 10 per week
