- Documents discharge planning contacts in progress notes, communicates with therapists and the treatment team about updates on the status of discharge planning.
- Participates in problem-solving regarding abrupt changes in discharge, coordinates and communicates the discharge plan.
- Coordinates discharge appointments and communicates to therapist and family if directed the treatment team.
- Works closely with Clinical Services and Utilization Review staff.
- Assists nursing department with prior authorizations.

QualificationsEducation/Experience
- Bachelor's Degree in a related field required; Social Work preferred
- Experience in discharge planning preferred
- Experience in a psychiatric setting a plus
