Identifies individual client needs and arranges services to address those needs. Helps determine need for public or private insurance benefits and other public assistance programs as needed by the client. In coordination with an Iowa Department of Public Health (IDPH) trained Benefits Specialist or other appropriate community provider assists client in applying for and maintaining eligibility for medical/insurance benefits and other public assistance programs. Provides outreach, crisis intervention and prevention/risk reduction education and medication adherence education specific to individuals with HIV/AIDS. Work is performed under the general supervision of the Program Director or Coordinator.
EXAMPLE OF DUTIES
Conducts a personal screening and initial assessment with the client and discusses resources and options available in the community. Develops individual service plan based on client needs and in accordance with well-defined Iowa Department of Public Health (IDPH) policies and procedures.Monitors and re-assesses client progress and needs according to client need and IDPH requirements.Works closely with key local and state service agency contacts to expedite access and remove barriers to information, services, employment, substance abuse treatment, mental health services, public benefits, housing and other client needs.Connects clients to appropriate services and treatments in accordance with their service plan. Transports clients to medical or other appointments as needed, either locally or out of town. Advocates for clients who are denied services for which they are eligible.Determines potential eligibility for public assistance programs and provides assistance in applying for local, state and federal public benefits programs. Assists client in maintaining eligibility for public assistance programs. Assists client in finding and maintaining appropriate and safe housing using community housing contacts and all available funding options. Determines potential eligibility for the AIDS Drug Assistance Program (ADAP) and provides assistance in referring client to an IDPH trained Benefits Specialist for applying for and renewing ADAP certification within the timeframes required by the funders.Participates in community-based meetings and LGBTQ events as a representative of the County and the Ryan White Program.May answer incoming calls, assist walk-in clients, and follow-up with new referrals as needed.Prepares letters, reports and general statistics as required by the Program Director or Coordinator. Schedules appointments and conducts personal visits in the client’s home, at the Ryan White Program office, homeless shelters, domestic abuse shelters, hospital, jail, medical facility or office and other community locations as necessary. Completes referrals for services as requested by the client and follows-up with the client to ensure satisfaction.Accurately completes all client documentation and contact notes within required timelines and in accordance with IDPH requirements using appropriate IDPH databases. Attends all required trainings and in-services and maintains annual Ryan White Program certification from IDPH.
