MENTAL HEALTH CARE MANAGER

JF&CS is seeking a Mental Health Care Manager to provide high quality, compassionate care to clients, founded on the empowerment model of service. The professional in this role has an opportunity to make a profound impact on the lives of individuals with mental health challenges and their families.

Through building a rapport with their clients and providing ongoing case management and care coordination, the Mental Health Care Manager helps their clients in many ways: exploring housing options; applying for, maintaining, and understanding their benefits and insurance; learning about and accessing new programs and resources; providing coaching and counseling around independent living skills; and communicating and coordinating effectively with family members and other care providers.

The Mental Health Care Manager role is both highly independent and collaborative. The workday includes:

  • Evaluating the strengths and needs of adults living with mental health challenges
  • One-on-one meetings with clients – both in the community and virtually
  • Educating clients and their families about options and resources
  • Creating and managing client-centered plans of care and adapting plans as needed
  • Facilitating client and family/care team meetings and relationships
  • Building relationships with community service providers, mental health professionals and other stakeholders
  • Documenting care plans and care management
  • Advocating for and maintaining appropriate ongoing services and benefits.

This is a full-time, Monday-Friday position. Schedule may be flexible and is a hybrid work environment between community visits, the Waltham office, and remote work.


ABOUT YOU

Ideal candidates for the role of Mental Health Care Manager bring a mix of clinical expertise, understanding, and flexibility to meet the needs of clients no matter their life circumstances.

A successful candidate is:

  • Keenly aware of the cognitive, physical, mental, and emotional capacities of adults with mental health diagnoses
  • Flexible and able to manage both ongoing care processes and unexpected challenges
  • Deeply familiar with community resources, entitlements, and health insurance options and requirements
  • An empathic listener that can create trusting relationships with clients and families.
  • An excellent team member and enthusiastic about a collaborative professional environment
  • A leader who can communicate effectively with clients, family members, other professionals and members of the care team.

Requirements include:

  • Degree in a social work or related discipline, plus 2+ years’ professional experience working with adults with mental health diagnoses in a community or medical setting preferred; or 5+ years relevant experience.
  • A valid driver's license and a reliable, insured vehicle.
  • Proficiency with Microsoft Office Suite, especially Word and Outlook

  • LICSW supervision towards social work licensure; access to free and low-cost CEUs
